Jewish Family Service of San Diego is seeking a Resettlement Case Manager to provide direct case management to refugee clients, coordinate referrals, and maintain precise records. This onsite role reports to the Resettlement Program Manager and requires strong organization, attention to detail, and cultural sensitivity.

You will collaborate with the Refugee Resettlement team to ensure timely documentation, program compliance, and successful client outcomes across CA.

Qualificações

• Valid CA driver’s license, insurance, reliable vehicle, and willingness to travel for meetings and client transport.
• Social services experience with case management.
• Experience working with refugee/immigrant populations.
• Fluency in client target language strongly desired.
• Excellent communication and time-management skills.

Responsabilidades

• Provide case management to eligible refugee/immigrant clients, including internal and external referrals, while tracking client progress toward goals.
• Assess and coordinate referrals to local service and housing providers, San Diego HHSA, and other partners as needed.
• Assist clients in enrolling in eligible benefits and orienting them to services.
• Advocate to help clients identify and access necessary services.
• Represent clients to help them communicate with providers.
• Advise clients on the necessity and purpose of services they receive.
• Conduct community outreach and presentations to develop support systems for eligible populations.
• Participate in collaborative meetings related to case management services.
• Manage data and documentation per program guidelines.
• Maintain case files with QA by the Program Manager.
• Other duties as assigned by supervisor.
