Job summary

Jewish Family Services of Western New York is seeking an Intensive Case Worker to support refugee and immigrant clients with complex needs. The role involves coordinating care, arranging transportation, and connecting clients to health and social services in a culturally sensitive manner.

You will work with the Refugee Services Team to develop individualized care plans, advocate for rights, and document services. Flexibility in hours and strong collaboration with providers are essential.

Qualifications

• Minimum of an associate degree in a human services related discipline.
• Experience providing direct services to refugees and immigrants.
• Experience linking clients to a broad range of essential, community-based services.

Responsibilities

• Provide services to refugees and immigrants including housing inspection, transportation, and access to benefits.
• Monitor and coordinate assessments and services per grant guidelines.
• Develop individualized, culturally responsive service plans and referrals.
• Coordinate interpretation services to ensure equal access.
• Document client contacts and services; maintain case notes and files.
