Job summary

Kodiak Area Native Association (KANA) is seeking a Referred Care Coordinator to support patient referrals and coordination of specialty care. You will serve as the main contact for patients, coordinate with providers and vendors, and help patients schedule and navigate referred care while ensuring timely follow-up and accurate records.

Work with care teams, vendors, and community partners to reduce barriers to care and improve patient outcomes.

Qualifications

• High school diploma or GED required.
• Three years of experience in healthcare, public-facing or patient services.
• Knowledge of healthcare operations and patient referral processes preferred.

Responsibilities

• Serve as a primary contact for patients regarding referral appointments.
• Coordinate referrals with specialty providers and external vendors.
• Assist patients with scheduling and navigation of referred care processes.
• Connect patients with insurance, travel, and support resources when appropriate.
• Monitor referral workflows and follow up on pending referrals and appointments.
• Maintain documentation in electronic health records and referral systems.
• Communicate with patients, providers, and external organizations to ensure continuity of care.
• Identify barriers to attending appointments and assist in resolving them.
