Job summary

Anchorage Neighborhood Health Center is seeking a Referral Coordinator to join our interdisciplinary care team in Anchorage. You will coordinate care for panel patients, focusing on those facing barriers to navigation, manage referrals end-to-end, and identify community resources to support timely access to care.

The role involves collaboration with outside providers, and support for patients across various needs including homelessness, pregnancy, HIV, mental health, and chronic diseases.

Qualifications

• One to two years of experience in a health-related or customer service field, or similarly transferable work experience.
• High School Diploma or GED.
• Bilingual preferred.

Responsibilities

• Coordinates care for patients before, during, and after visits to ensure timely access to needed care.
• Manages referral processes from start to finish and follows up on incomplete referrals.
• Coordinates with medical facilities, specialists, and community services to ensure continuity of care.
• Communicates with patients about care plans and resources; supports coordination with PCP or care team.
• Participates in team meetings and coverage for other coordinators as needed.
