St. Croix Health is seeking a full-time Referral Unit Coordinator to support providers and patient care staff. Responsibilities include entering patient messages into the EMR, managing referrals, and scheduling appointments while maintaining accurate records.

The role emphasizes timely referrals, coordination with external facilities, and attention to patient care needs with consideration for diverse populations.

Responsibilities

• Coordinates activities to support provider and patient care staff including monitoring provider schedules, add-ons, cancellations, no-shows and overbooks, and schedules patient appointments for testing, referrals or follow-up.
• Sends referrals to external health care facilities for services not provided at St. Croix Health.
• Monitors internal/external referrals and coordinates next steps with patient.
• Make referral appointments including scheduling preliminary appointments/testing.
• Demonstrates ability to provide care or service adjusting approaches to reflect developmental levels and cultural differences of population served.
