Job summary

North Shore Community Health is seeking a Referral Coordinator II to manage the specialty referral process and support seamless patient transitions to specialty care. You will coordinate referrals for complex cases, engage with patients to address challenges, and liaise with specialists across the North Shore region.

The role reports to the Manager of Patient Experience and emphasizes accuracy, communication, and follow-through to ensure timely care for diverse patient populations.

• Courteous, efficient, and effective communication with patients and teams.
• Strong interpersonal and communication skills, with ability to mentor junior staff.
• Proficiency with Microsoft Office; EPIC and healthcare tech is a plus.
• Maintain strict confidentiality and align with NSCH values.

Responsibilities

• Oversee the referral coordination to specialty services.
• Coordinate appointments for specialty services as requested by the ordering provider.
• Collect proper data for each referral before sending to the specialist.
• Provide feedback to the manager and care team.
• Be the point of contact for hospitals and specialty offices.
• Respond to referral status inquiries and provide accurate information.
