Job summary

Southwest Network, Inc. is seeking a referral coordination assistant to track, coordinate, and communicate outgoing member referrals across multiple portals, ensuring timely delivery and accurate medical-record updates.

The role emphasizes HIPAA confidentiality, EMR software proficiency, and cross-site travel. Evening or weekend hours may be required; CPR/First Aid training and a valid Arizona driver’s license are expected.

Qualifications

• Associate degree or completion of a medical records certification program preferred.
• Minimum 1 year of experience in healthcare, preferably with collection and disbursement of medical records.
• Must maintain a valid Arizona state fingerprint clearance card.
• Must be trained in CPR, First Aid and Therapeutic Options.
• Must have and maintain a valid Arizona driver’s license.
• Must be at least 21 years of age.

Responsibilities

• Tracking, coordination, and communication of outgoing member referrals.
• Assembling information concerning a member’s clinical background and referral needs.
• Ensuring referrals are sent to the receiving provider in a timely manner.
• Navigating multiple referral portals to ensure documentation is uploaded and reflected in the medical record.
• Upholding confidentiality of verbal and written information in accordance with HIPAA and policy.
• Other duties as assigned.
