Hartford HealthCare at Hartford Hospital seeks a referral coordination professional to ensure patients are cleared for specialty services visits. You will act as the point person for questions on orders, referrals, and prior authorizations, coordinating with hospitals, physicians, plans, vendors, and patients.

In this role you will support clinical staff with administrative components of referrals, manage outgoing referrals to agencies, and contribute to Care Plans objectives.

Qualifikationen

• Associate's degree in ambulatory environment or equivalent.
• Five years' experience with referral processes.
• Familiarity with EPC EHR system.

Aufgaben

• Ensure patients are cleared for specialty services office visits.
• Serve as point person for questions on orders/referrals/prior authorizations/workques; liaise between hospital, physicians, health plans, vendors and patients.
• Manage all outgoing referrals to appropriate agencies to support Care Plan goals.
