Job summary

Northern Counties Health Care, Inc. is seeking a Referral Coordinator to ensure timely access to specialty care by coordinating, processing, and tracking patient referrals.

You will work with patients, clinical staff, insurers, and community partners to verify completeness, obtain authorizations, and document referrals in the EHR. This role may provide medical reception coverage during high-demand periods.

Qualifications

• High school diploma or equivalent required.
• Experience in a medical office, referral, scheduling, patient access, or healthcare administrative role preferred.
• Experience with electronic health records and insurance authorization processes preferred.
• Knowledge of medical terminology preferred.
• Strong organizational skills and ability to manage high-volume, time-sensitive work.
• Strong written, verbal, and interpersonal communication skills.
• Ability to maintain confidentiality and exercise sound judgment.
• Ability to work independently while collaborating with a multidisciplinary team.
• Ability to provide medical reception coverage as needed.

Responsibilities

• Processes incoming and outgoing referrals accurately and within established timeframes.
• Reviews referral requests for completeness and obtains missing information.
• Coordinates with providers, clinical staff, patients, and community organizations to facilitate referral completion.
• Determines whether prior authorization or additional documentation is required and coordinates steps.
• Sends records and supporting documentation to receiving organizations via secure methods.
• Assists patients with referral-related questions, scheduling, and next steps.
• Tracks referrals from initiation through completion and follows up as needed.
• Maintains accurate referral information in the EHR and other systems.
• Maintains patient confidentiality and handles protected health information.
• Provides medical reception coverage during absences or increased demand.
