Central Coast VNA & Hospice is seeking a dedicated Hospice RN Case Manager in Salinas, CA. You will coordinate interdisciplinary care, provide skilled nursing support focused on comfort and quality of life, and guide patients and families through transitions and discharge planning.

Requirements include California RN license, 1 year in home health or hospice, CPR certification, and strong collaboration with physicians and care teams. A reliable vehicle and driver's license are preferred.

Salary Range: $60.92 To $66.08 Hourly

VNA is looking for a Hospice RN Case Manager .

Job Summary: The Hospice Case Manager collaborates with physicians and the interdisciplinary care team to identify, coordinate, and address the home care needs of patients, families, and caregivers. The Hospice Case Manager provides skilled nursing care and supportive services using the nursing process, with a focus on patient comfort, safety, and qualify of life.

They refers and coordinates other disciplines and specialty services and facilitates transitions to other levels of care or services, as needed. Manages patient care in accordance with agency standards, applicable regulations, and payer requirements. Provides education and support to patients, families, and caregivers and prepares patients and families for discharge when appropriate.

Status:Full-Time

Education/Experience/Skills/Qualifications:

• Active Registered Nurse license in the California
• Minimum of one year nursing Home Health or Hospice experience required
• Knowledge of home health, hospice, palliative care, and other agency services preferred
• Current CPR certification
• Ability to work collaboratively with physicians, families, caregivers, and the interdisciplinary team members
