Job summary

Palms Medical Group is seeking a Case Manager to deliver exceptional customer service and coordinate patient referrals. The role supports continuity of care by managing referral workflows, securing authorizations, and acting as a liaison among patients, providers, and external specialists.

Responsibilities include handling patient communications, coordinating referrals, and assisting with community resource navigation.

Qualifications

• Must be organized, self-starter and detail oriented.
• Ability to work independently and as part of a team.
• Multitasking capability and knowledge of referral sources in the geographical area.

Responsibilities

• Provide professional and courteous patient support within PMG policies.
• Answer, screen and route calls; operate a multi-line phone system; document interactions in the EHR.
• Coordinate referrals: prioritize, collect demographics/insurance info, communicate next steps to providers and patients.
• Identify community resources and assist patients in navigating language or social barriers to referral completion.
• Follow EHR-based referral tracking and record reports; ensure timely processing of referrals.
• Obtain prior authorizations and present medical information to justify necessity.
