Claremedica Health Partners LLC is seeking a Referral Coordinator to schedule and manage referrals, communicate with patients and providers, and navigate the health-care system. You will maintain referral records, verify carrier requirements, and facilitate prior approvals as needed.

The role emphasizes patient service, documentation, and collaboration with the Referral Management Supervisor to ensure timely referrals and smooth care transitions.

Qualifications

• High school diploma or GED or equivalent experience.
• Minimum of 1 year of experience processing referrals; MA or front desk experience preferred.
• HIPAA regulations knowledge; maintains confidentiality.
• Exceptional oral and written communication, time management, and organizational skills.
• Proficient with Microsoft Office Suite including Word, PowerPoint, Outlook and Excel.
• Must be able to type at least 40 WPM.
• Ability to communicate professionally with employees, patients and others.
• English proficiency; bilingual is a plus.

Responsibilities

• Schedules, solves problems, communicates, and coordinates referral appointments with outside specialists upon receipt of the Referral Order.
• Maintains knowledge of health plan carrier requirements for referrals and approvals.
• Provide excellent service in person or by phone; schedule initial and follow-up appointments.
• Maintain tracking and documentation on referrals to promote awareness and patient safety.
• Assemble information on patient background and referral needs.
• Provide clinical information to specialists per referral guidelines.
• Coordinate with review organizations and insurers for prior approvals.
• Present medical information to financial services to maximize reimbursement; request records as needed.
• Remind patients about appointments; assist with barriers (interpreters, transportation, prescriptions).
• Be system navigator and advocate for patients with carriers.
• Identify community resources and establish provider relationships.
• Perform misc administrative tasks as required by Referral Management Supervisor.
