Job summary

PAM Health, LLC is seeking a Clinical Navigator to be the face of our hospital, cultivating referral relationships and managing an assigned territory. You will meet patients at their toughest times and collaborate with the admissions team, nursing staff and payers to facilitate referrals and admissions.

The role requires building strong stakeholder relationships, providing patient-centric service, and contributing to program development and quality improvement initiatives within the care

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.
• Minimum of two years’ experience in the designated field of license.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Analyzes referral and admission data and coordinates with admissions and nursing staff.
• Provides feedback for program development and quality improvement related to customer service and satisfaction.
