Qualifications

• High school diploma or equivalent required.
• Minimum 1 year of experience processing referrals or related front-desk/clinical work.
• HIPAA compliance and confidentiality
• Excellent verbal and written communication
• Professional, courteous demeanor with patients and staff
• Proficient with Microsoft Office and basic computer tasks.

Responsibilities

• Schedules and coordinates referral appointments with outside specialists.
• Maintains knowledge of health plan referral requirements and authorizations.
• Provides excellent customer service in person and by phone.
• Schedules initial and follow-up appointments with specialists and imaging may be required.
• Maintains referral documentation for team awareness and patient safety.
• Transfers necessary medical information to providers and financial services as needed.
• Communicates clearly with patients about referrals, expectations, and barriers.
• Acts as a navigator for patients, addressing problems and coordinating resources.
• Works with service providers to identify community resources as needed.
