Job summary

CareSouth Medical & Dental seeks a Referral Coordinator to manage, track and monitor outgoing and internal referrals, ensuring patient safety and care coordination. You will assist in improving access to care and maintaining relationships with health services across the Baton Rouge area.

Responsibilities include coordinating referrals, maintaining the referrals database, and ensuring EMR documentation aligns with quality and safety standards.

Qualifications

• Minimum of high school diploma.
• Preferred Certified Medical Assistant or Licensed Practical Nurse or associates in a health field.
• Minimum of three years of experience with working referrals.
• Demonstrates a high level of skill at building relationships and providing excellent customer service.
• Inhibits a strong attention to detail and accuracy.
• Has the ability to utilize computers for data entry and information retrieval.
• Demonstrates knowledge of the referral process for a variety of insurance plans.
• Demonstrates success in researching and resolving complex issues.
• Must have strong written and verbal communication skills.
• Must have pleasant telephone etiquette; ability to deal tactfully and effectively with patients; must have strong computer skills, use of Microsoft applications and familiarity with electronic health records.

Responsibilities

• Manage referrals, track and monitor to ensure timely and safe patient care.
• Maintain and update the database of referral contacts for the service area.
• Coordinate referrals with hospitals, clinics, and providers outside CSMD as needed.
• Ensure documentation is accurately entered in the EMR.
• Commit to timely outreach to patients and health agencies on referral status.
• Attend trainings and departmental meetings to stay aligned with policies.
