Job summary

St. Elizabeth Healthcare is seeking a Care Management Referral Coordinator to manage the Care Management Referral Line, coordinate referrals, and support the SEP Grant Funded Transportation Assistance program.

The role involves telephonic and face-to-face care management interventions, chart review, and collaboration with care teams. The coordinator will educate patients, link families to community resources, and ensure timely documentation.

Qualifications

• Bachelor's degree or equivalent experience in a related field.
• Minimum 2+ years in a health-related or community resources setting.
• Valid driver’s license, reliable transportation, current auto insurance.
• Working knowledge of Excel, Windows and Outlook; ability to learn additional computer skills.

Responsibilities

• Answer all incoming calls to the Care Management Referral Line and resolve issues.
• Review referrals for appropriate routing using clinical judgment.
• Conduct initial assessments to route referrals to the correct team member.
• Coordinate with patients and families to meet needs and educate about resources.
• Schedule and manage appointments for the Care Management Team.
• Link patients to community resources and monitor progress.
• Educate patients on care plans, timelines, and goals.
• Collaborate with providers and community supports to coordinate services.

Education

Bachelor’s degree in a related field

Associate’s degree with equivalent experience
