Responsibilities

Program Leadership & Team Supervision: Direct, supervise, and evaluate a multi-disciplinary team of resource specialists delivering housing, employment, health navigation, and essential support services.
Inter-Departmental Service Coordination: Establish seamless referral workflows with facility-based transition teams and community outreach staff for immediate, face-to-face handoffs.
Community Network & Provider Expansion: Build and maintain partnerships with state agencies, housing authorities, unions, healthcare networks, and non-profit groups.
Field Safety & Supervision Alignment: Enforce safety boundaries and coordinate field deployments with outreach staff and probation/parole officers.
Performance Metrics & Contract Oversight: Monitor service delivery velocity and outcomes to ensure alignment with standards.
