Nehemiah Center for Urban Leadership Development is seeking a Reentry Navigation Coordinator to serve as the central point of contact for participants and the administrative backbone of the Reentry program. This role will run weekly check-ins, coordinate support services, and ensure program data, reporting, and rounds coverage are running smoothly.

Responsabilités

• Run weekly check-ins with residents and participants.
• Maintain secure case notes on each participant’s needs, accomplishments, and concerns.
• Conduct intakes, orientations, and participant assessments.
• Assist with participant exit reviews and data logging.
• Coordinate referrals and connect participants to outside support services.
• Serve as liaison between referring organizations and Nehemiah.
• Assist with lease signing and documentation with housing director.
• Share advocacy opportunities and participant progress data with the Reentry Navigation Coordinator.
• Maintain data tracking systems and produce weekly impact reports.
• Compile monthly program reports for Vice President of Reentry Services for leadership review.
• Prepare end-of-year program numbers for accounting and city reporting.
• Schedule staff meetings and ensure timely submission of timesheets.
• Assist with grant reporting and funder relationships as assigned.
• Maintain an accurate database of participant contact information.
• Schedule property rounds and coordinate coverage.
• Coordinate cross-role check-ins on property conditions and resident needs.
