Job summary

Helping Hands in Lincoln City seeks a Case Manager for our reentry program. You’ll build supportive relationships with participants, help them access resources, and support goal setting toward stable housing.

This full-time role requires trauma-informed, data-driven, and person-centered approaches. You’ll coordinate with partners, document progress, and maintain confidentiality as you guide participants toward independence.

Qualifications

• Experience in case management or social services preferred.
• Ability to work collaboratively with participants and partners.
• Trauma-informed approach to support participants.
• Strong documentation and confidentiality practices.

Responsibilities

• Build rapport with participants and learn about their needs through conversations and records review.
• Meet with participants monthly to develop and update IRPs, set goals, review progress, and identify next steps.
• Support participants in understanding and meeting program requirements.
• Help participants access housing, healthcare, SNAP, DMV, employment supports, and other services.
• Coordinate with healthcare providers, counselors, DHS workers, parole officers and community partners.
• Respond to crises using trauma-informed de-escalation and connect participants with timely resources.
• Develop and maintain a network of community resources including medical and mental health services and legal assistance.
• Build relationships with landlords and housing authorities; stay informed about housing opportunities.
• Provide practical education and resources for securing and maintaining permanent housing.
• Maintain accurate documentation and participant confidentiality.
