Job summary

Changing Perceptions in Washington, DC seeks a Re-entry Case Manager to support justice-impacted individuals in reintegration. You will conduct intake, develop ISPs, coordinate services, and advocate for participants within housing, employment, and healthcare systems. A trauma-informed, empathetic approach and bilingual abilities are a plus.

The role reports to the Chief Program Officer and offers a competitive salary, healthcare, and a hybrid work schedule with in-office days on Tue-Thu.

Qualifications

• Experience in case management, social work, or re-entry services.
• Commitment to dignity and empowerment of justice-involved individuals.
• Strong organizational and multitasking abilities.
• Excellent verbal and written communication skills.
• Familiar with DC-area resources.

Responsibilities

• Conduct participant intake and assessment to understand goals and barriers.
• Develop ISPs focused on employment readiness, housing stability, and health.
• Provide ongoing case management, referrals, scheduling, and transportation support.
• Navigate internal and external services, including public benefits and workforce programs.
• Maintain accurate documentation per policy and funder requirements.
• Respond to urgent needs with crisis intervention and connect to long-term resources.
• Collaborate with partners to ensure wraparound support.
• Advocate for participants within criminal justice, housing, employment, and health systems.
