Job summary

Kinexus Group in Cassopolis, MI is seeking a Case Manager to support the Pathway Home 6 reentry program for justice-involved individuals. You will lead intake, develop IDPs, and coordinate services pre- and post-release, with regular jail visits and collaboration with partners.

You will track progress, maintain records, and liaise with jail staff and service providers to promote successful employment outcomes and reduced recidivism in Michigan communities.

Qualifications

• Associate’s degree or equivalent in human services or related field (Bachelor’s preferred).
• At least two years of case management or direct services experience.
• Strong interpersonal skills and motivational interviewing ability.
• Comfort working in correctional and community settings.
• Proficient in Microsoft Office; familiarity with electronic case management systems.

Responsibilities

• Lead intake and assessment; develop and manage IDPs.
• Coordinate in-jail visits twice weekly.
• Connect job seekers with housing, legal aid, mental health, and other services.
• Track progress toward self-sufficiency and reduced recidivism.
• Maintain confidential participant records and documentation.
• Enter participant data into LS/CMI system accurately.
• Attend weekly case coordination and staff performance meetings.
