Job summary

Leon County Sheriff's Office is seeking a Reentry Case Manager to perform interviews, assessments, and referrals for inmates based on needs evaluations. The role involves developing inmate case plans and coordinating training programs for successful re-entry into the community.

Responsibilities include monitoring progress, coordinating with internal/external partners, and documenting data. A strong emphasis is placed on communication, data entry, and adherence to SOPs and core values.

Qualifications

• High school diploma or GED required.
• Bachelor's degree in Social Work, Sociology, Psychology, Criminology or Social Sciences preferred.
• Experience in criminal justice field preferred.
• Proficient in Microsoft Office.
• Strong data tracking and report writing skills.
• Valid driver's license with satisfactory driving record.
• Strong interpersonal skills with offender population.
• Program facilitation experience highly desirable.

Responsibilities

• Perform interviews and assessments to determine offenders' risks and needs.
• Work within correctional environment with inmates at Leon County Detention Facility.
• Prepare case history reports and recommend program assignments.
• Document progress and adjust assignments as needed.
• Counsel offenders on personal, employment, education, and family needs.
• Facilitate reentry programs and transition planning.
• Develop and monitor client transition plans and collaborate with partners.
• Report data weekly/monthly/quarterly/annually.
• Communicate with colleagues to reduce duplication.
• Meet licensing and in-service trainings.
