Qualifications

Job description

The primary duties of the Reentry Case Manager are case management, providing assessments, and proper referral for services to inmates based upon evaluation and assessment of needs. The employee must demonstrate the ability to develop, evaluate, and analyze program needs and data collection pertaining to inmates. The incumbent in this position will also assess the progress of individuals in the facility, including post-release, and will coordinate and implement inmate training programs and evaluate the program’s effectiveness. The incumbent will also create inmate case plans to optimize success of the inmate while in detention and for their transition back into the community. This position adheres to the agency's core values of honesty and integrity, accountability, teamwork, trust, respect, and commitment to excellence. To be successful in this position, the employee must demonstrate competency in agency Standard Operational Procedures and General Orders, conflict resolution, team building skills, strategic planning, application of transformational leadership, data entry and retrieval skills, time management skills, possess above average computer skills, excellent communication skills (both written and oral), interpersonal skills, customer service skills, and problem- solving skills.

This position is funded partially or in whole with Special Revenue Funds or Grants and is contingent upon the continued availability of these funds. Should funding cease or be reduced, this position may be eliminated, and the employee may not appeal the elimination of the position under the Career Service Act.

  1. Perform interviews and assessments to determine offenders' risks and programming needs.
  2. Ability to work within a correctional environment, specifically with inmates assigned to the Leon County Detention Facility.
  3. Ability to prepare case history reports and recommend program assignments.
  4. Ability to review work and document necessary information of all assigned offenders to determine progress or necessity of changing assignments/classification.
  5. Ability to assist the classification unit for caseload needs.
  6. Ability to counsel offenders on personal problems, employment resources, vocational skills, educational training needs, family problems and institutional adjustment.
  7. Ability to facilitate programs to offenders which are designed to assist with successful re-entry back into the community following release from incarceration.
  8. Ability to develop, monitor, and amend client transition plans in line with re-entry programs and services.
  9. Ability to establish a collaborative network of internal and external partners to provide support, pre and post release services for offenders, that can involve family members, community resources and social services agencies. Internal partners include, but are not limited to, law enforcement, school resource, CSMB, youth services, and intelligence.
  10. Communicates daily/weekly/regularly with fellow case managers, Detention Deputies, and Behavioral Health Care Staff (YesCare and Apalachee Center) to streamline and reduce instances of service duplication.
  11. Ability to identify and recommend programming, social services, referrals, and focused case management on identified high risk offenders.
  12. Ability to collect, organize, and report statistical data (as required) on a weekly, monthly, quarterly, and annual basis.
  13. Ability to perform proper research on treatment, transitional, and employment services.
  14. Ability to facilitate classes and programs.
  15. Ability to research correctional self-improvement programs that assist with controlling and overcoming problems.
  16. Ability to understand and carry out oral and written instructions.
  17. Ability to research and build community partnerships.
  18. Perform any additional assignments as deemed necessary by supervisors.
  19. Ability to work closely with others as a team.
  20. Ability to effectively communicate both orally and in writing.

  22. Ability to meet requirements and maintain required licensures and certifications as well as agency required in-service training.

  24. Ability to report to work timely, consistently and as scheduled.
  25. Ability to perform all functions of the job classification without posing a direct threat to the health or safety of other individuals in the workplace.
Performance Measures
  1. Maintain and manage an offender caseload, as assigned.
  2. Schedule, manage, complete, and document, assessments, case plans, and contact meetings as instructed by the Reentry Programs Supervisor.
  3. Report deliverables to Reentry Programs Supervisor in weekly case staffing meetings.
  4. Enter data into Apricot as designated per department expectations.
  • High School Diploma or equivalent (GED) required.
  • Bachelor's degree from an accredited college or university required; majors in Social Work, Sociology, Psychology, Criminology, or Social Sciences preferred; AND
  • Experience working in the criminal justice field preferred; AND
  • Demonstrate skills in Microsoft Office products (i.e. excel, word, publisher. etc.) preferred.
  • Demonstrate skills with data tracking and report writing.
  • Possess and maintain a valid driver's license with a satisfactory driving record.
  • Demonstrate successful interpersonal skills, particularly with the offender population.
  • Program facilitation experience preferred and highly desirable.
  • Knowledge of core Detention practices relating to reentry services highly desired.
  • Equivalent combinations of education and/or experience sufficient to successfully perform the essential duties of the position will be considered.
  • Must be able to lift, push, carry or pull a minimum of 50 pounds independently. Position has an increased risk of occupational exposure to hazardous and infectious materials.
  • Duties are performed within a Detention Facility and requires varying degrees of standing, sitting, and walking throughout the workday.
