Job summary

Bridgeways is seeking a Reentry Case Manager for the Jail Transition Program in Everett, WA. You will independently manage a caseload of 20–30 clients, meeting them in jails and in Snohomish County to plan successful reentry and connect them with housing, health care, and other resources.

The role emphasizes independent work, relationship-building, problem-solving, and timely EMR documentation, with a daytime Mon–Fri schedule and home office in Everett.

Qualifications

• Experience providing case management or direct support to people navigating complex needs.
• Comfortable meeting new people and building rapport with individuals from diverse backgrounds.
• Experience working within jail and criminal justice settings.
• Ability to manage multiple clients, appointments, deadlines, and follow-ups independently.
• Proactive about connecting people with resources and troubleshooting barriers.
• Able to travel to required work locations throughout Snohomish County.
• Eligible to work in the United States.

Responsibilities

• Meet with clients in local jails and Snohomish County to assess needs and plan for reentry.
• Connect clients with housing, behavioral health and substance use treatment, health care, employment, benefits, transportation, and other resources.
• Develop transition plans, monitor progress, troubleshoot barriers, and follow up to keep things moving.
• Build relationships and coordinate with jail staff, providers, government agencies, and partners.
• Advocate for clients and provide support when challenges arise.
• Maintain timely case notes and documentation in the EMR.
• Manage your own schedule, appointments, priorities, and follow-ups.
