Job summary

Bridgeways seeks a Jail Transition Case Manager in Everett, WA to support individuals reintegrating after jail. You will conduct needs assessments, develop transition plans, and connect clients with housing, employment, and mental health resources to reduce recidivism.

The role requires strong advocacy, collaboration with law enforcement and social services, and precise documentation in electronic medical records while maintaining HIPAA standards.

Qualifications

• Bachelor’s degree in social work, psychology, criminal justice, or related field; Master’s degree preferred.
• Minimum two years’ experience in case management, social services, or criminal justice (criminal justice system preferred).

Responsibilities

• Maintains reliable attendance and adheres to scheduling.
• Conducts comprehensive assessments of clients’ needs, risks, strengths.
• Develops and updates transition plans addressing housing, employment, health care needs.
• Connects clients with housing, job training, mental health, and substance abuse resources.
• Maintains ongoing case management with follow-ups and progress monitoring.
