Job summary

Delphi Rise, part of Delphi Drug & Alcohol Council Inc., seeks a full-time case manager to support individuals returning to the community from incarceration in Monroe County. The role focuses on dignity, relationship-building, and helping clients access housing, employment, health care and healing services.

Responsibilities include managing 40–50 clients, coordinating with state and county agencies, maintaining records, developing service plans, and facilitating support groups.

Qualifications

• Experience in case management and coordinating services with multiple agencies.
• Strong written and verbal communication skills.
• Ability to maintain accurate client records and develop service plans.

Responsibilities

• Manage a caseload of 40 to 50 clients, helping access benefits, housing, employment, healthcare and other services.
• Coordinate services with NYS Department of Corrections, Probation, courts, and partners.
• Maintain timely case notes and client records.
• Develop individualized service plans aligned with client goals.
• Facilitate support groups (CBT, parenting, domestic violence, anger management).
• Biweekly follow-ups with clients; participate in Task Force meetings.
