Job summary

180 Degrees, Inc. is hiring a full-time Re-entry Case Manager at Clifton Place in Minneapolis, MN to support men transitioning from incarceration. You will provide intensive case management, help navigate housing, income, and community resources, and build realistic exit plans in a supportive, trauma-informed setting.

The role emphasizes accountability-based practices, strong boundaries, and empathy, with 40 hours per week onsite and a focus on long-term stability for residents.

Qualifications

• Bachelor’s degree in human services or related field.
• Two years of related case management experience in criminal justice or homelessness.
• Valid driver’s license and transportation for community travel.
• Ability to clear background screening.
• Strong communication and documentation skills.
• Trauma-informed, youth-centered approach.
• Proficiency with computers and data systems.

Responsibilities

• Provide intensive, ongoing case management to residents at Clifton Place.
• Help residents comply with court-ordered conditions and legal requirements.
• Assist access to housing, employment, healthcare, and other resources.
• Develop realistic exit plans focusing on housing and income.
• Apply trauma-informed, person-centered practices to reduce barriers.
• Maintain safety, structure, and program expectations.
• Build trusting relationships with residents navigating reentry.

Education

Bachelor’s degree in human services, psychology, social services, social work, or related field

Two years of related case management experience in criminal justice, community re-entry, or homelessness

Valid driver’s license and ability to travel to partners

Strong written and verbal communication skills

Proficiency with computer systems and databases

Trauma-responsive and youth-centered practice

Care coordination, advocacy, and collaborative problem-solving
