Treatment Alternatives for Stronger Communities (TASC) is seeking a full-time Case Manager to support community reentry initiatives through addiction intervention, recovery support services, and coordination with multiple agencies. The role focuses on helping clients and families achieve established goals and navigate behavioral health resources within the context of the correctional system.

Formación

• High school diploma or GED required.
• Valid driver’s license with access to a personal vehicle.
• CADC certification within two years may be required by contract.
• Pass an IDOC Background check.
• Proficient in Microsoft Office suite.
• Experience in group facilitation.
• Knowledge of ASAM criteria, DSM-V related to substance disorders, or ability to acquire.
• Comfort working in a correctional setting and with serious offenses.
• Strong verbal, written, and interpersonal communication.
• Good time management and ability to meet deadlines.

Responsabilidades

• Assist individuals in achieving service plan goals through coordination with behavioral health, public agencies, and community resources.
• Assess and monitor progress relative to parole and service plans.
• Complete intake packets for enrollment and participation.
• Provide referrals and crisis intervention for identified needs.
• Interview individuals and review records to assess program suitability.
• Collaborate with internal/external partners and participate in staff meetings.
• Identify and refer individuals with behavioral health needs.
• Maintain documentation including screenings, service plans, and progress notes.
• Ensure confidentiality of information.
• Maintain progress toward service plan benchmarks.
