Job summary

TASC is hiring a full-time Case Manager to support clients involved in the criminal legal system dealing with substance use and behavioral health challenges. You will assess, plan, refer, and coordinate services to promote recovery and stability within the ACJS division.

Responsibilities include intake, referrals, documentation, and collaboration with partners to ensure seamless, person-centered care while adhering to program standards and ethical guidelines.

Qualifications

• High school diploma or GED required.
• CADC certification within two years may be required.
• Must pass IDOC Background check.

Responsibilities

• Conduct comprehensive assessments and develop service plans.
• Facilitate intake and enrollment into TASC programs.
• Provide referrals and crisis intervention for client needs.
• Collaborate with community partners for coordinated care.
• Apply DSM-5 and ASAM criteria in referrals.
• Maintain timely documentation, progress notes, and reports.
• Monitor progress toward service goals and reporting compliance.
• Participate in staff trainings and multi-disciplinary discussions.
• Support clients in navigating systems and services.
