TASC Treatment Alternatives for Stronger Communities in Illinois seeks a full-time Case Manager to support clients with addiction intervention, AODA counseling, and recovery services, coordinating with agencies, resources, and families to reach goals.

The role may involve high-risk caseloads and working within correctional settings; responsibilities include intake, referrals, progress monitoring, documentation, and collaboration with partners to ensure compliance and successful reentry.

Qualifications

• Assess and monitor progress per client-centered service plan.
• Complete intake packets for enrollment and participation.
• Refer and coordinate behavioral health needs with partners.

Responsibilities

• Assess, evaluate and monitor individuals' progress relative to the service plan and parole conditions.
• Complete intake, enrollment, and participation paperwork.
• Provide referrals and crisis intervention for identified needs.
• Collaborate with internal and external partners and participate in staffing.
• Document progress notes, reports and discharge summaries.
