TASC is seeking a full-time Case Manager for the Community Reentry program in Illinois. You will deliver addiction intervention, AODA counseling, and recovery support, coordinating services across agencies to help clients meet goals.

Responsibilities include outreach, intake, referrals, and collaboration with partner organizations to ensure comprehensive care, while maintaining confidentiality and documenting progress.

Competenze

• High school diploma or GED required.
• Valid driver's license required.
• CADC certification within two years of employment may be required.
• Must pass IDOC Background check.
• Proficiency in Microsoft Office suite.
• Experience in group facilitation.
• Knowledge of ASAM placement criteria and DSM-V or ability to learn.
• Comfort in correctional settings and with individuals convicted of serious offenses.
• Ability to lead group discussionsflexibly.
• Strong verbal, written, and interpersonal communication skills.
• Good time management and deadlines tracking.

Mansioni

• Assist individuals in achieving service plan goals through direct services and coordination of behavioral health, public agencies, and community resources.
• Assess and monitor progress relative to service plans and parole requirements.
• Complete intake packets for program enrollment and participation.
• Provide referrals and crisis intervention for identified needs.
• Interview individuals, review records, and consult with professionals to assess suitability for participation.
• Collaborate with internal and external partners and participate in staff meetings and supervision.
• Identify and refer individuals with behavioral health needs.
• Prepare and submit internal and external program reports.
• Maintain documentation of screenings, enrollments, service plans, progress notes, and discharge summaries.
