Job summary

Community Health Associates is seeking a Peer Support Specialist to work with adults and children, offering unconditional, nonjudgmental support and leveraging lived recovery experience. The role focuses on helping members start improving quality of life through goal setting and community resource navigation.

This position collaborates with the treatment team, provides skills training spanning daily living, and conducts both individual and group sessions on site and at home.

Qualifications

• High school diploma or GED required.
• Must be at least 21 years old.
• Must have firsthand knowledge of behavioral health or substance abuse, having received treatment personally or as a family member.
• Peer Support training within 6 months of employment.

Responsibilities

• Provide peer support to engage members in a recovery partnership and assist them in identifying hopes and dreams for the future and finding solutions to problems.
• Provide rehabilitation and support services to aid in meeting treatment goals through individual and group sessions on site and in members' homes.
• Identify, navigate, and use community resources with members.
• Transport members to and from appointments as appropriate.
• Assist members in setting personal recovery goals.
• Collaborate with the treatment team to determine steps to achieve goals.
• Provide skills training in nutrition, budgeting, personal finance, organization, time management, and relapse prevention.
• Inform members about service options available through referrals.
• Coordinate with the treatment team regarding ongoing care.
• Follow directives from program manager and professionals.
