GUIDANCE CARE CENTER INC. seeks a Peer Specialist to support medical services, help with treatment plans, and empower clients in recovery. You will conduct face-to-face visits, liaise with the Comprehensive Care Team, and assist with referrals and documentation.

The role emphasizes peer-led recovery, trauma-informed care, and collaboration with community services to promote wellbeing for individuals with severe mental illness.

Formación

• High School Diploma or equivalent is required
• Must hold current Peer Certification as CPPS and/or CPRS or obtain within 1 year of employment
• Experience with crisis intervention is preferred

Responsabilidades

• Serve as a member of the Comprehensive Care Team (CCT)
• Develop person-centered Wellness and Recovery plans with the individual and CCT
• Encourage peers to pursue their Wellness and Recovery goals
• Apply Motivational Interviewing to engage service recipients
• Provide health promotion activities to individuals with SMI and community participation
• Document services in the medical record and participate in case conferences
