Groups is a leading outpatient provider specializing in substance use disorder (SUD) treatment, committed to underserved communities and recovery support through compassionate, collaborative care delivered in person at our Manchester, NH location.

The Recovery Support Coordinator helps a defined panel of 160-180 members with health-related social needs, care coordination, and engagement across clinical, social, and community settings, reporting to the Recovery Operations Supervisor.

Qualifications

• Strong interpersonal and communication skills.
• Knowledge of recovery-oriented care and motivational interviewing.
• Ability to navigate community resources and support systems.
• Excellent organizational and time-management skills.
• Ability to maintain confidentiality and professional boundaries.
• Proficiency with digital tools and EHR systems.
• Willingness to work evenings or flexible hours.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trust with members and assist in achieving recovery goals through coaching and motivational interviewing.
• Assist members with identifying goals and engaging in recovery activities.
• Support re-engagement in treatment services through outreach.
• Provide in-person, virtual, and telephonic support based on needs.
• Maintain caseload and coordinate care pathways for high-risk members.
• Connect members to healthcare, behavioral health, social services, and community resources.
