Job summary

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency (Arizona-based with programs in Oregon) seeking a Continuing Care Peer.

This role delivers peer support, outreach, care planning, and resource linkage for individuals facing behavioral health challenges, homelessness, or crises, both on-site and in the community. The Continuing Care Peer helps with deflection, sobering services, and transitions to appropriate care levels, while ensuring follow-up, advocacy, and

Qualifications

• High school diploma or GED required.
• Minimum six months of recovery from substance use or mental health challenges.
• Experience in outreach, peer support, homeless services, or behavioral health preferred.
• Driver’s license (valid and in good standing) required.
• Clear 39-month motor vehicle record required.
• Peer Support Specialist certification from the Oregon Health Authority required or within 90 days of hire.

Responsibilities

• Provide peer support, outreach, care planning, and resource linkage onsite and in the community.
• Support individuals during deflection encounters, sobering services, and transitions to appropriate levels of care.
• Ensure follow-up, advocacy, and continuity of services.
