Job summary

Community Bridges, Inc. (CBI) seeks a Certified Peer Specialist to support individuals in recovery from mental health and substance use disorders, offering experience, strength, and hope to patients, families, and significant others during care transitions.

The role collaborates with the treatment team to connect patients with appropriate levels of care and to involve family and social supports in discharge planning.

Qualifications

• Highschool diploma or GED required.
• Associate Degree or higher preferred.
• Minimum of six months of recovery from substance use and/or mental health disorders required.
• Health Care related experience where prior experience would provide exposure to behavioral and medical crisis situations preferred.

Responsibilities

• Assist in transition to the most appropriate level of care with the treatment team.
• Coordinate care with all parties involved in the patient’s case.
• Ensure family and other social supports identified in the treatment plan are involved in discharge planning.
