Job summary

Community Bridges, Inc. (CBI) seeks a Continuing Care Peer to provide community-based outreach, support, navigation, and care coordination for individuals facing behavioral health challenges, substance use, homelessness, or crisis situations.

This role relies on lived recovery experience and includes peer support, outreach, and helping clients transition to appropriate levels of care, with strong follow-up and advocacy.

Qualifications

• High school diploma or GED required.
• Minimum six months of recovery from substance use and/or mental health challenges.
• Experience in outreach, peer support, homeless services, or behavioral health preferred.
• Driver’s license (valid and in good standing) required.
• Clear 39-month motor vehicle record required.
• Peer Support Specialist certification from the Oregon Health Authority required or able to be obtained within 90 days of hire.

Responsibilities

• Provide community-based outreach, support, navigation, and care coordination.
• Deliver peer support and resource linkage onsite and in the community.
• Assist individuals during deflection encounters, sobering services, and transitions to care.
• Ensure follow-up, advocacy, and continuity of services.

Job Summary

The Continuing Care Peer is an individual with lived experience in recovery and who provides community-based outreach, support, navigation, and care coordination for individuals experiencing behavioral health challenges, substance use, homelessness, or crisis situations. Hourly Rate: $19/HR - $22.80/HR
