Job summary

Community Bridges, Inc. (CBI) seeks a Peer Support Specialist to provide experience, strength, and hope to individuals and families, helping minimize the emotional impact of treatment and support recovery.

You will help coordinate care and assist in transition planning with the multidisciplinary team. The role requires a high school diploma or GED, with some college preferred, and a minimum of six months recovery from substance use or mental health disorders.

Qualifications

• Six months of recovery from substance use or mental health disorders required.
• Certified Peer Support Specialist certification (for Screening and Assessment programs only) required or preferred.
• Valid identification required.

Responsibilities

• Support patients and families using experience, strength, and hope to minimize emotional impact and aid recovery.
• Assist in transitioning to appropriate level of care with the treatment team and discharge planning.
• Coordinate with RN, PCT, BHC and supervisors to ensure continuity of care.
