Qualifications

Responsibilities

• Supports person-centered planning and implements interventions per Restoration Center Support Plan.
• Fosters a welcoming, accessible environment within the Restoration Center.
• Provides support, education, and advocacy during transitions such as hospital discharge or relocation to independent living.
