Job summary

Vinfen in Massachusetts is seeking a Restoration Center Recovery Coach who has lived experience with addiction and/or co-occurring mental health disorders. The role provides intensive case management and 1-on-1 supports in the Sober Support Unit to help individuals build Recovery Support Plans and achieve community tenure.

You will work with a multidisciplinary team, coordinate services, assist with housing, benefits, and transportation, and help clients navigate health care and social services

Qualifications

• Lived experience with addiction and/or co-occurring mental health disorders with at least 3 years in recovery.
• Minimum of 2 years care management or related experience with people with psychiatric conditions or SUD preferred.
• Training and/or demonstrated experience in working with peers assisting in recovery and life goals.
• Preference for bi-lingual/bicultural applicants.

Responsibilities

• Performs outreach to newly referred members to encourage program participation.
• Coordinates orientation and Restoration Support Plan completion.
• Designs crisis management and relapse prevention plans with the SSU team.
• Assists clients in navigating health care, mental health, SUD treatment, and social supports.
• Helps members improve daily living skills and access services.
• Provides temporary transportation assistance to essential appointments.
• Assists with obtaining benefits, housing, and health care.
• Collaborates with ESP/MCIs and outpatient providers on safety and crisis plans.
• Fosters recovery, empowerment, and connections to peer support and self-help.
