Job summary

Rosecrance Ware Center Rockford, IL is seeking dedicated staff to advocate for designated clients and participate in addictions treatment services with families. The role supports recovery in daily living while ensuring ethical and compliant care.

Responsibilities include transporting clients, conducting home visits, documenting progress, and collaborating with a multidisciplinary team to deliver treatment services and ensure client safety.

Qualifications

• High School Diploma or GED required.
• Ability to drive a multi-passenger vehicle in various conditions.
• Must qualify as RSA per Illinois DoH Rule 132.
• Excellent written and oral communication skills.
• Ability to quickly establish rapport.
• Skilled in behavior management techniques.
• Independent worker who collaborates with a team.
• Valid driver’s license and ability to meet agency insurance requirements.
• Must have own transportation for designated positions.
• Belief in Rosecrance mission.

Responsibilities

• Transport clients to and from agency and activities as needed.
• Conduct home or community visits to support progress in treatment.
• Document significant findings related to treatment goals.
• Advocate for clients in courts, clinics, or social services as needed.
• Participate in client graduation events.
• Support case management during transitions to higher levels of care.
• Assist with housing, food, or other needs.
• Coordinate with outreach staff to ensure consistent care.
• Perform CLIA-waived testing and related quality control.
• Complete documentation in client records.
• Be an active member of the treatment team and attend meetings.
• Adhere to Rosecrance Compliance Program and Code of Conduct.
• Deliver excellent customer service to clients and staff.
