Job summary

Hamilton Health Clinic in Harrisburg, PA seeks a Housing Case Manager to provide formal case management and housing support to opioid-dependent individuals, promoting recovery and independent living goals.

The role coordinates housing-related activities, connects clients with community resources, and supports treatment and recovery plans while maintaining documentation and compliance with regulations.

Qualifications

• Bachelor's degree in Psychology, Social Work, Education, Criminal Justice, Social Sciences, Addictions, or related field preferred.
• In lieu of a Bachelor's degree, a minimum of ten (10) years of experience in the human services field with enrollment in and pursuit of a Bachelor's degree within six months.
• At least one year of related experience preferred; other combinations of education, training, and experience may be considered.
• PA State Police Criminal Record Check, Child Abuse History Clearance, and FBI Criminal Record Check required.
• Valid Pennsylvania driver’s license, reliable transportation, and current automobile insurance.

Responsibilities

• Provide service coordination including case management, care coordination, client advocacy, and action plan development.
• Meet regularly with clients to implement housing plans and stability goals.
• Assist with budgeting, employability, family support services, counseling referrals, and community connections.
• Maintain relationships with landlords, housing agencies, and community resources for housing opportunities.
• Document service coordination activities in the continuum of care in a timely manner.
• Review and modify Service Plans to support treatment and recovery goals.
• Arrange advocacy services for housing, court involvement, or family support needs.
