Job summary

Hamilton Health Center in Harrisburg, PA seeks an Intensive Case Manager I to provide comprehensive case management, care coordination, and advocacy for individuals receiving recovery and supportive services.

You will work with clients, healthcare providers, and community partners to coordinate services, reduce barriers to care, and support recovery journeys through individualized service planning and ongoing care coordination.

Qualifications

• Bachelor's degree in Human Services or a related field.
• Strong interpersonal, communication, and organizational skills.
• Basic computer and data entry proficiency.
• Fundamental understanding of healthcare services and medical terminology.
• CPR/First Aid certification (preferred).
• One year of experience in a medical or physician office setting (preferred).
• Knowledge of Joint Commission standards (preferred).
• Bilingual in Spanish, Nepali, or French (preferred).

Responsibilities

• Provide comprehensive case management, care coordination, and advocacy for clients receiving recovery services.
• Develop, implement, and update individualized service plans and monitor progress.
• Coordinate primary care and behavioral health services within the care continuum.
• Assess client needs and coordinate holistic care plans.
• Maintain ongoing communication with clients, families, and support systems.
• Connect clients with housing, legal, transportation, and community resources.
• Document service coordination activities and maintain accurate records.
• Build collaborative relationships with providers, community agencies, and civic organizations.
• Participate in meetings, trainings, and continuing education.
• Maintain certifications and complete annual training; ensure confidentiality and regulatory compliance.
• Perform other related duties as assigned.
