Qualifications

• High School Diploma and 3 years of relevant work experience in behavioral health.
• Bachelor’s degree in a social sciences field is acceptable.
• Understanding of mental illness and its impact on functioning.
• Valid driver’s license and insurable driving record; willingness to travel.

Responsibilities

• Coordinates assessments, treatment planning or crisis intervention services.
• Facilitates healthcare, behavioral health and other services as part of care coordination.
• Provides training and linkages for use of community resources.
• Monitors symptoms and service delivery to meet client needs.
• Assists clients in obtaining services for basic necessities.
• Helps clients access self-help groups and social support.
• Coordinates crisis management and stabilization; communicates with the treatment team.
