Job summary

Bridgeway is hiring a Wellness Specialist (Case Manager) in New Jersey to join the Community Support Services team. You will provide direct support, skill development, and resource linkage to individuals with serious mental illness to help them integrate into the community of their choice.

The role includes crisis intervention and collaborative care with licensed staff. Requires an Associate, Bachelor's, or Master's degree in a helping profession and relevant experience; valid driver's license

Qualifications

• Associate degree in a helping profession with two years of community-based behavioral healthcare experience; may substitute Psychiatric Rehabilitation Practitioner certification for one year of experience.
• Bachelor's degree in social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice.
• Master's degree in a helping profession as above.

Responsibilities

• Provide resource development, skill development, and direct support for people with serious mental illness enrolled in CSS to integrate into the community.
• Contribute to rehabilitation needs assessments and update individualized recovery plans (IRP).
• Perform crisis intervention as needed and coordinate services with the team.
