Job summary

Bridgeway Behavioral Health Services, Inc. in Clinton Township, NJ seeks a Wellness Specialist (Case Manager) to join the Community Support Services team.

The full-time position runs Tuesday–Saturday, 9:00am–5:00pm, and focuses on guidance, skill-building, and recovery-oriented support for individuals with serious mental illness. You will contribute to rehabilitation planning, crisis intervention, and coordinated care, while delivering counseling, health education, and evidence-based practices

Qualifications

• Two+ years of experience in a community-based behavioral healthcare setting.
• Associate degree in a helping profession from an accredited college.
• Bachelor's degree in a helping profession.
• Master's degree in a helping profession.
• Certification as Psychiatric Rehabilitation Practitioner may substitute for one year of experience.

Responsibilities

• Create and maintain a wellness and recovery-oriented environment using person-first language and evidence-based practices.
• Maintain compliance with internal and external policies, regulations, laws and high ethical and professional standards.
• Contribute to the Comprehensive Rehabilitation Needs Assessment (CRNA) and IRP development.
• Perform therapeutic rehabilitative skill development as outlined in CSS Regulations.
• Offer services to meet clients' needs as identified in CRNA and IRP.
• Perform crisis intervention as indicated.
• Coordinate and manage services in conjunction with the team.
• Provide supportive counseling, medication education, mental health education and early crisis intervention.
• Engage people receiving services in the treatment process.
• Utilize IMR, WRAP and other health wellness curricula to emphasize wellness in recovery plans.
• Provide health literacy support to all team members and clients.
• 24-hour on-call coverage on a rotating schedule.
• Deliver education on recovery and wellness strategies within the team.
