Prairie Ridge is seeking a Peer Support Specialist to provide recovery-oriented care. You will assist patients with mental health needs and daily living activities in homes, offices, or community sites, while collaborating with a multidisciplinary team to support independence and safety.

Candidates should have lived experience with mental health services, a high school diploma or GED, and be prepared to obtain WRAP training within six months. Valid transportation and EHR skills are required.

Qualifikationen

• Experience living with mental health services for self is required.
• Knowledge of and experience with adults with mental illness is required.
• Peer Support certification training within the first six months of employment.
• WRAP training or willingness to obtain WRAP training after hire.
• Strong written and oral communication and record-keeping skills.
• Proficiency with electronic health records and related tech.
• Valid driver's license, insurance, and reliable transportation.

Aufgaben

• Provide direct care and basic living support in home, office, or community locations.
• Coordinate daily living activities and support housing options.
• Collaborate with a multidisciplinary team and participate in planning.
• Assist patients with paperwork and entitlement applications.
• Deliver crisis response under supervision when needed.
• Maintain patient confidentiality and adhere to CARF policies.
• Engage in quality improvement activities and data collection.
