Qualifications

• Bachelor's degree in a human services field required or equivalent experience.
• Associate’s degree in a human services field plus two years of direct services experience.
• High school diploma plus four years of direct human services experience.
• Equivalent combination of training and experience may be substituted.

Responsibilities

• Secures information to develop consumer treatment plans and quarterly reviews.
• Provides supportive counseling in 1:1 or group formats.
• Maintains and documents case records and service plans per regulatory guidelines.
• Facilitates individualized services and discharge-planning goals for consumers.
• Administers substance abuse services and conducts urinalysis/breathalyzer tests as directed.
