Aufgaben

• Secure information contributing to the individual's situation and update treatment plans quarterly.
• Provide 1:1 or group supportive counseling to modify attitudes and behaviors.
• Maintain up-to-date case records and develop measurable service plans.
• Document case notes and discharge planning activities.
• Monitor medications and coordinate medical appointments as directed.
• Assist in coordinating social, vocational, medical and other activities for consumers.
