Qualifications

• Bachelor's degree in a human services field or equivalent
• Two years of full-time experience in direct services with mental disabilities or related field
• High school diploma with four years of direct human services work experience

Responsibilities

• Secures information and completes consumer treatment plans including quarterly reviews
• Provides supportive counseling in 1:1 or group formats
• Maintains accurate case records and develops measurable service plans
• Assists with discharge planning and coordinates services with providers
• Implements cognitive-behavioral techniques to support recovery and daily living skills
• Monitors medications and accompanies consumers to medical appointments
• Engages in crisis assessment and intervention as needed
• Participates in training, meetings, and shift coverage as assigned
